Our Nurse Practitioner, Shiane Filmer FNP-C,  will work closely with you to understand your symptoms, medical history, and health goals, then design a customized treatment plan that supports your unique needs and lifestyle.

After booking your appointment, you’ll receive access to our secure Self Check-In system, which becomes available 7 days before your visit. This online form allows you to complete your medical history, social history, and assigned screening questions in advance. It’s important that this is filled out prior to your appointment, as it allows us to provide the most personalized and efficient care possible.

At your visit, you’ll meet with our Nurse Practitioner, Shiane Filmer, who will review your intake forms, talk through your symptoms, discuss your goals, and explain the hormone therapy options available to you. If appropriate, Shiane will order lab work to assess your current hormone levels and other relevant health markers. Your next step will be a follow-up appointment to review your lab results in detail and develop a customized treatment plan that aligns with your health goals.

You’ll be cared for by Shiane Filmer, ARNP, a board-certified Family Nurse Practitioner with a clinical background in critical care and plastic surgery. A lifelong Gig Harbor local, Shiane has been proudly serving the community since 2017.

Her diverse experience and passion for building lasting, trusting relationships allow her to take a whole-person approach to midlife wellness. She takes the time to listen, understand your symptoms, and create a plan that supports both short-term relief and long-term vitality.

To begin hormone therapy with Shiane, all patients must have an established primary care provider and be current on age-appropriate health screenings, including Pap smears and mammograms. 

After your initial consultation, Shiane will provide personalized lab orders based on your symptoms, history, and wellness goals. 

No, we do not prescribe hormone pellets as part of our hormone therapy approach. Pellets are small implants placed under the skin that release hormones over several months. While commonly marketed for convenience, pellets are not FDA-approved and have limited long-term safety data supporting their use.

Additionally, one of the main drawbacks of pellet therapy is the inability to easily adjust dosing once the pellet is implanted. At Gig Harbor Aesthetics and Wellness, we prioritize safe, flexible, and evidence-based care. Your treatment plan may include FDA-approved options such as transdermal patches, topical creams, or oral medications, which allow for more precise, individualized hormone dosing and easier adjustments as your needs evolve.

Yes. Once you are registered as a patient, you will have access to a secure patient portal through our electronic health record system, AthenaHealth. The portal provides a convenient way to stay connected and manage your care. You’ll be able to view your visit summaries, access lab results, review prescriptions, and send secure messages to your care team whenever you need to get in touch.

Note: The AthenaHealth system is used exclusively for wellness services such as hormone therapy & weight management. All scheduling and correspondence for cosmetic or skincare appointments is handled separately through our aesthetic platform, Mangomint.

No, we do not currently accept insurance for hormone therapy visits. All consultations and follow-up appointments are self-pay. However, your insurance may cover your lab work and prescriptions, depending on your individual plan and benefits. We are also happy to provide a detailed receipt (also known as a superbill) upon request, which you can submit to your insurance provider for possible reimbursement. Coverage varies, so we recommend checking with your insurance carrier regarding out-of-network benefits.

No, there is no set age at which you must stop hormone therapy. The decision to continue or discontinue Hormone Therapy should be made on an individualized basis, taking into account your symptoms, health status, and personal preferences.

Contrary to popular belief, routine discontinuation of Hormone Therapy at age 65 is not supported by evidence. As stated in the 2022 position statement from The North American Menopause Society:

“The recommendation to routinely discontinue systemic hormone therapy in women aged 65 years and older is neither cited or supported by evidence, nor is it recommended by the American College of Obstetricians and Gynecologists or The North American Menopause Society.”

For many women, especially those in good health who continue to benefit from treatment, Hormone Therapy can be used safely beyond age 65 with proper monitoring. At Gig Harbor Aesthetics and Wellness, Shiane will work with you to reassess your therapy regularly and ensure it remains appropriate for your evolving needs.
